In the Spouse of Contact Applicant section, complete the following fields:

Title	Select the spouse’s title from the drop-down list.
First Name	The spouse’s current legal first name.
Middle Names	Any middle names the spouse may have.
Family Name	The spouse’s current family name.

If the spouse has changed their name, complete the following details:

Original First Name	The first name on the spouse’s birth certificate.
Original Middle Names	Any middle names on the spouse’s birth certificate.
Original Family Name	The family name on the spouse’s birth certificate.
Other Name Used?	Select 'Yes' if the spouse has previously used another name. certificate.

Complete the birth details:

Date of Birth	The date of birth of the spouse, in DD/MM/YYYY format.
State/Territory of Birth	Town or suburb where the spouse was born.
Country of Birth	Select the country where the spouse was born.
Contact Phone	Enter the spouse's daytime contact telephone number.
Select Yes or No from the drop-down list to answer the question Previously Lived Interstate? Note: This field appears for only some jurisdictions.
Select Yes or No from the drop-down list to answer the question Is this spouse also an applicant?
