Complete documentation details

At some stage in the FHOG application process documentary evidence must be sighted to support the application. This can be a passport, birth certificate, drivers licence, etc. Details of the evidence can be put in the Document Reference field (or Name of person sighting supporting evidence field for NSW).

In the Documentation section:

  1. Select Yes or No from the drop-down list to answer the question Document Evidence Sighted?
  2. Enter the Stat Dec Signed Date. Note: This field appears for only some jurisdictions.
  3. Enter the Document Reference details sighted, such as the passport number. For NSW, enter the Name of person sighting supporting evidence.
  4. Select Yes or No from the drop-down list to anser the question Boost Addendum Received?
  5. For NSW and QLD SRO applications only, enter the Application Complete Date. This is the date on which a complete application was lodged, including all forms, certifications and documentation (leave this field blank if the application is still incomplete).
