Answer Addendum Questions
Applications lodged in QLD jurisdication on or after 18/11/2003 must be accompanied by an 'Addendum' form. The answers to the addendum questions are entered at the bottom of the main application form.
The application is not considered complete, and cannot
be processed further, until all the addendum questions have been answered.
To answer the addendum questions:

Answer each addendum question at the bottom of the page by clicking either the Yes or No option button.
All addendum questions must be answered in order for the application to be saved.
